"Artificial pancreas", providing the natural normal glycemic profile in the patient with DM

It is known that the methods of traditional insulin therapy turned out to be ineffective for the stable maintenance of the glycemic profile in the patient with Diabetes mellitus (DM), inherent in a healthy person, in the range of glycemic level [72–110] mg/dl, that protect him from the development of the dangerous hypoglycemia, as well as the typical late complications of diabetes. To this dual task of insulin therapy with limitations in the chronically acceptable level of glycemia in the patient with DM to the maximum and to the minimum, a third task is added - to create the individual postprandial excesses above normoglycemia in him for providing with his glucose energy supply. It was supposed to solve this problem of fulfilling the conditions of the noted three tasks of insulin therapy by technical methods using an automatic insulin pump, which was, a priori, called an "artificial pancreas", believing that it can completely replace the physiological insulin function of the natural pancreas in a patient with DM. However, it turned out that even the most advanced modern automatic insulin pumps in reality solve only one of these tasks of insulin therapy - the inadmission of the hypoglycemia state in a patient, but cannot provide the simultaneous fulfillment of its second and third tasks: the absence of chronic hyperglycemia in him and the presence of postprandial slight exceedings above normoglycemia. Their best achievement in limiting hyperglycemia at present (as with traditional insulin therapy) is only inadmissibility of the patient's hyperglycemic level above 180 mg/dl, and only for a limited time. In this paper it was proposed the improvement of an automatic insulin pump by changing its operate system to the dynamic limits of normoglycemia, inherent in a healthy person, based on the original physiologically adequate mathematical model of glycemic dynamics. This made it possible to modify the automatic insulin pump and made it possible to solve all three noted tasks of insulin therapy on its basis, i.e. turn it into a real "artificial pancreas".
